# PMDD and asthma: how they interact

PMDD and asthma share a hormone and inflammation link: childhood asthma is tied to a higher risk of PMDD, and asthma itself often gets worse in the days before a period. The two are usually treated on separate plans, but tracking both together can show your doctor the real pattern.

What it is

PMDD

Premenstrual asthma

A pattern of worse asthma symptoms before a period

## At a glance

- Premenstrual asthma is a documented pattern: symptoms or peak flow readings worsen before a period in roughly 30 to 40 percent of women with asthma, by some estimates.
- Childhood asthma is linked to a higher risk of meeting criteria for a premenstrual disorder like PMDD as an adult, per a 2023 cohort study of 6,524 young women.
- Progesterone appears to reduce how responsive a key airway receptor is, which may help explain why breathing worsens before a period, separately from the mechanisms thought to drive PMDD mood symptoms.
- No treatment has been shown to clearly help both conditions at once; doctors treat PMDD and asthma on separate, evidence-based plans.
- Tracking a peak flow diary alongside a daily mood chart, lined up by cycle day, is what shows a doctor the real pattern.

The asthma-PMDD link and the premenstrual asthma pattern are backed by a cohort study and several reviews. Treatment interactions come mostly from small, older trials and case reports, so they're suggestive rather than settled.

| Compared on | PMDD | Premenstrual asthma |
| --- | --- | --- |
| What it is | A DSM-5-TR mood diagnosis tied to the luteal phase | A pattern of worse asthma symptoms before a period |
| Main symptoms | Irritability, low mood, anxiety, rejection sensitivity | More wheezing, coughing, chest tightness, lower peak flow |
| Timing | Last week before a period, eases within days of bleeding starting | Can start days before bleeding and sometimes continue into it |
| How it's confirmed | Daily symptom ratings over two cycles | Daily peak flow readings tracked against cycle day |
| Suspected mechanism | A different brain response to normal hormone shifts | Hormone-linked changes in airway inflammation and a key receptor |
| Who treats it | GP, gynecologist, or psychiatrist | GP or respiratory specialist, sometimes with a gynecologist |

## How are PMDD and asthma connected?

PMDD and asthma are different conditions, but they share a biological thread: inflammation and a strong reaction to normal hormone shifts. [PMDD](https://niyora.com/pmdd/) itself is a different reaction to normal hormone changes, not a sign of abnormal hormone levels.

A 2023 cohort study in Nature Mental Health followed 6,524 girls and young women for years and found:

- Childhood asthma was linked to a higher risk of meeting criteria for a premenstrual disorder as adults, with an adjusted risk ratio of 1.20

- The link looked stronger for PMDD than for milder PMS

- Childhood food allergy showed a similar pattern

One possible mechanism: progesterone affects beta2-adrenoceptors, a receptor that helps keep airways open. In a small crossover study, giving women progesterone reduced how responsive this receptor was, an effect estrogen didn't produce (Chest, 1997). If something similar happens naturally as progesterone falls before a period, it could help explain why breathing worsens in that window, through a separate pathway from the brain-based mechanisms thought to drive PMDD mood symptoms.

This doesn't show one condition causes the other, just that the same hormone drop may affect mood in PMDD and the airways in asthma.

## Does asthma get worse before your period?

Yes, for a meaningful share of women with asthma. Premenstrual asthma (PMA) describes a documented pattern: symptoms or peak flow readings worsen in the days before a period and ease once it starts. When an existing condition consistently worsens before a period like this, it's sometimes called [premenstrual exacerbation](https://niyora.com/pme/).

Estimates vary, but several reviews put the share of women with asthma who notice this pattern at roughly 30 to 40 percent (Journal of Allergy and Clinical Immunology, 2003; Drugs, 2001; Journal of Asthma, 2004). The exact cause isn't settled. Researchers have looked at airway inflammation, changes in beta2-adrenoceptor function, and fluid retention in the bronchial lining, without one clear answer.

A 2017 systematic review of 20 studies found that women with premenstrual asthma tend to share certain features:

- Older age and a longer history of asthma

- More severe asthma overall

- A higher body mass index

- Aspirin-sensitive asthma

- Painful periods, PMS, shorter cycles, or longer bleeding

None of these guarantee you'll have premenstrual asthma, and having none of them doesn't rule it out. They're patterns seen across groups, not a checklist for one person.

If your chest feels tighter, you're coughing more, or you're reaching for your inhaler more in the same days each month, that timing is worth writing down for your doctor.

Right now

If your chest and your mood are both bad right now

- 1 Use your inhaler as prescribed. Don't wait to see if the tightness passes on its own.
- 2 Sit up straight, drop your shoulders, and breathe out for six counts before you breathe in again.
- 3 Say both things out loud: your chest is tight and you feel terrible. Treat them as two separate problems.
- 4 Write down what happened and how your breathing felt, so a doctor can see the pattern later.

Niyora walks you through this when it happens:

- Calm down
-
- See it another way
-
- Choose what to say

## How common is it to have both?

There's no single number for how many women have both PMDD and asthma, but a 2023 cohort study (Nature Mental Health) and a 2017 systematic review (Expert Review of Respiratory Medicine) give a sense of scale.

In the 2023 cohort of 6,524 young women, 19.3 percent met criteria for a premenstrual disorder overall, and a history of childhood asthma raised that risk by about 20 percent. Separately, several reviews estimate that up to 30 to 40 percent of women with asthma notice a premenstrual worsening pattern in their breathing.

Put together, the 2023 cohort study and the 2017 review suggest that having asthma may raise the odds of also noticing a cyclical mood pattern, though no study has measured this overlap directly. Two inflammation-linked conditions seem to travel together here, rather than one causing the other, and tracking is what makes that overlap visible.

## How do treatments interact?

Doctors usually treat PMDD and asthma with separate, well-established plans, since no single treatment has been shown to fix both at once.

For PMDD, guidelines point to SSRIs (sometimes taken only in the luteal phase), certain hormonal contraceptives, and CBT, with a doctor deciding what fits your history. For asthma, including premenstrual flares, the usual inhalers and controller medicines remain the first approach, and most women's symptoms respond to them even in the days before a period.

| Approach | What it targets | What the evidence shows |
| --- | --- | --- |
| Usual asthma medicines | Asthma, including premenstrual flares | Most premenstrual asthma responds to standard treatment |
| Added estrogen before a period | Premenstrual asthma | A placebo-controlled crossover trial found no clear difference from placebo in peak flow or symptoms (Pharmacotherapy, 2003) |
| Prostaglandin-blocking drug (meclofenamate) | Premenstrual asthma | Improved peak flow early in the premenstrual phase, not in the final days before or during a period (JACI, 1987) |
| Combined contraceptives or GnRH analogs | Premenstrual asthma, based on case reports | Suggested by case reports, not yet confirmed in large randomized trials |
| SSRIs (daily or luteal-phase only) | PMDD mood symptoms | Standard guideline treatment for PMDD; no evidence it changes asthma either way |

None of this is routine care yet for the asthma side, and any hormone-based option needs a specialist's judgment. If you take asthma medication and your doctor is considering a hormonal option for PMDD, mention both conditions so they can see the full picture.

## What helps in the days before your period?

When your chest feels tight and your mood feels awful on the same day, it helps to treat them as two separate problems. Managing the asthma comes first, since breathing is the one that can turn serious fast.

What can help in the next ten minutes:

- Use your inhaler as prescribed. Don't wait to see if the tightness passes on its own.

- Sit up straight, drop your shoulders, and breathe out for six counts before you breathe in again.

- Say both things out loud: "my chest is tight and I feel terrible." Naming them separately stops you from blaming one for the other.

- Write down what happened, including how your breathing felt and what you said or did, so a doctor can see the real pattern later.

Niyora is a PMS and PMDD support app. If you want somewhere to put the mood side while you manage your breathing, Niyora's Moon lets you write what happened, see it from another angle, calm down, and draft a reply you can hold for 20 minutes.

## How do you track both?

Tracking is what turns a vague sense of "this always happens" into something a doctor can act on.

For the mood side, the [Daily Record of Severity of Problems](https://niyora.com/tools/drsp-chart/) (DRSP) is the standard chart used to confirm PMDD. You rate a set of symptoms daily for at least two cycles. For the breathing side, a simple peak flow diary, morning and evening readings plus inhaler use, does the same job for asthma.

Putting both on the same calendar, lined up by cycle day, is what shows a pattern instead of two separate complaints. If your peak flow dips and your mood scores rise in the same days each month, that's useful evidence either way.

If you're not sure where you stand, [this quiz](https://niyora.com/quiz/) can give you a sense of how much PMS or PMDD symptoms are affecting you, as a starting point before you track in more detail. A [PMDD specialist](https://niyora.com/care/pmdd-specialist/) or your regular doctor can use [this process](https://niyora.com/care/how-to-get-diagnosed/) to confirm what's going on, and bring in a respiratory doctor if the asthma pattern needs its own workup.

When to see a doctor

If your asthma needs more inhaler use around the same days each month, or your mood symptoms take a week of every month, bring two months of notes covering both to your doctor.

## Questions people ask

Can asthma cause PMDD?

+

Asthma doesn't cause PMDD, but childhood asthma is linked to a higher risk of developing PMDD later, according to a 2023 cohort study of over 6,500 young women. The link is statistical, not a guarantee, and other factors play a role too.

Why does asthma get worse before my period?

+

Premenstrual asthma seems tied to falling progesterone and estrogen affecting airway inflammation, and specifically to progesterone's effect on a receptor that normally helps keep airways open. The exact mechanism isn't settled, but up to 30 to 40 percent of women with asthma notice this pattern.

Should I change my asthma inhaler before my period?

+

Don't change your asthma medicine on your own. If you notice your asthma getting worse at the same point in your cycle every month, bring a peak flow diary to your doctor so they can decide whether your plan needs adjusting.

Is premenstrual asthma a real diagnosis?

+

Premenstrual asthma isn't a separate diagnosis on its own. It describes a documented pattern, worse asthma symptoms or lower peak flow before a period, confirmed by tracking readings against your cycle rather than by a single test.

Can treating PMDD improve my asthma symptoms?

+

There's no evidence that PMDD treatments like SSRIs improve asthma, or that asthma treatment improves PMDD. The two are usually treated on separate, doctor-guided plans, though mentioning both conditions helps your doctor see the whole picture.
