# PMDD and PCOS: how they interact

PMDD and PCOS are separate conditions with different causes, but they can happen in the same person and their symptoms can overlap. PCOS involves androgen and insulin levels that stay high most days of the month, while PMDD is a different reaction to the normal hormone drop in the week before your period. Tracking both over two cycles is the clearest way to tell what's actually happening in a given week.

## At a glance

- PMDD and PCOS are different conditions, and having PCOS doesn't cause PMDD.
- PCOS symptoms like acne, fatigue, and bloating can flare in the week before your period even though PCOS itself isn't cycle-dependent.
- Some treatments overlap (certain combined pills help both), but a doctor needs to weigh them together, since what helps one condition can affect the other.
- Tracking both for two full cycles is the clearest way to separate a PCOS flare from a PMDD week.
- If mood symptoms take over a week every month, or you have thoughts of hurting yourself, see a doctor and bring daily notes.

## Strength of evidence

Mixed evidence

Treatments for PMS and PMDD alone have moderate evidence (Cochrane reviews, network meta-analyses, RCTs), but no study here tested women with confirmed PCOS and PMDD together, so the comorbidity itself is poorly mapped.

| Compared on | PMDD | PCOS |
| --- | --- | --- |
| What it is | Mood disorder tied to the cycle | Hormone and metabolic condition |
| When symptoms show up | Luteal week only | Can happen most days of the month |
| Core mechanism | Brain's reaction to a normal hormone drop | Higher androgens, insulin resistance |
| How it's diagnosed | Two cycles of daily symptom tracking | Blood tests, ultrasound, cycle history |
| Typical mood symptom | Anger or rejection sensitivity premenstrually | Anxiety or low mood, any time of month |
| First-line treatment | SSRIs, some combined pills, CBT | Lifestyle changes, metformin, some combined pills |

## How are PMDD and PCOS connected?

PMDD and PCOS are two different conditions. Having one doesn't mean you have, or will develop, the other.

PCOS is a hormone and metabolic condition. Androgen levels run higher than typical, ovulation is often irregular or absent, and insulin resistance is common. Those changes are present most days of the month, not only before a period.

PMDD comes from how strongly the brain reacts to the normal rise and fall of estrogen and progesterone each cycle, not from abnormal hormone levels themselves.

A small 2016 trial found that a higher dose of a drug blocking progesterone's conversion to allopregnanolone eased core PMDD symptoms, while a lower dose did not. That's one small piece of evidence for a broader finding. PMDD is a different reaction to the normal hormone shift, not to abnormal hormone levels (Schmidt et al., 1998, NEJM; Dubey et al., 2017, Molecular Psychiatry).

The two conditions sit on different tracks: one about hormone and metabolic levels, one about brain sensitivity to a normal hormone drop. They can still coexist in the same body. For the fuller picture of the second one, see [what PMDD is and how it's diagnosed](https://niyora.com/pmdd/).

## Does PCOS get worse before your period?

PCOS itself doesn't follow a strict cycle pattern, but many of its symptoms can feel worse in the week before your period. A condition that's present all month but flares before bleeding is called premenstrual exacerbation, or PME. See [PME: when your period makes another condition worse](https://niyora.com/pme/) for more on this pattern.

Symptoms that often flare in the luteal week include:

- Acne breakouts

- Fatigue or low energy

- Bloating and breast tenderness

- Mood dips, irritability, or anxiety

- Heavier or less predictable bleeding

A meta-analysis of premenstrual symptom research found that stress and premenstrual symptom severity move together in general, and people with a core premenstrual disorder showed higher stress specifically in the luteal phase (BJPsych, 2025). No study has tested whether PCOS's added daily stress specifically makes the luteal week harder, but it's a plausible overlap.

Right now

When both flare in the same week

- 1 Name which symptom is loudest right now (skin, bloating, mood, fatigue) so you treat the actual problem.
- 2 If you're about to snap at someone, say you need 20 minutes, leave the room, and set a timer.
- 3 Eat something with protein before deciding anything big; low blood sugar sharpens everything else.
- 4 Write a few lines about what happened while it's fresh, so you can check later if it's a pattern.

Niyora walks you through this when it happens:

- Calm down

- See it another way

- Choose what to say

## How common is it to have both?

There's no solid number yet for how many women with PCOS also meet full criteria for PMDD. Most menstrual cycle research studies one condition at a time, so the overlap hasn't been mapped well.

One systematic review found that PMS and PMDD are both common among female athletes, but it didn't report how often the two occur in the same woman, so the overlap with PCOS specifically is still unmapped.

What's clearer is that PCOS and PMDD share some symptoms: irritability, low mood, anxiety, and fatigue. That overlap makes it easy to miss that both are happening, or to blame one for symptoms that belong to the other, which is part of why daily tracking across two full cycles matters more than judging a single week before her period. See [how PMS and PCOS interact](https://niyora.com/conditions/pms-and-pcos/) for the related, usually milder pattern.

## How do treatments interact?

Some treatments used for PCOS and PMDD overlap, but a doctor needs to weigh them against each other, not pick one in isolation.

| Option | What it's typically used for | Evidence for PMDD or PMS |
| --- | --- | --- |
| Combined oral contraceptives | PCOS (cycle regulation, lowering androgens) | A network meta-analysis found some formulations eased depressive and overall premenstrual symptoms |
| Non-contraceptive estrogen (patch or gel) | Not used for PCOS | A Cochrane review found modest evidence for easing PMS symptoms |
| Metformin | PCOS insulin resistance | Not studied for PMDD mood symptoms |
| SSRIs (daily or luteal-phase only) | PMDD mood symptoms | Recommended in treatment guidelines; timing is a doctor's call |
| Nutritional support (B6, calcium, zinc) | General premenstrual symptoms | RCTs found consistent benefit for psychological symptoms |

The same pill can pull in two directions. An anti-androgenic combined pill chosen for PCOS acne might not be the formulation with the strongest PMDD evidence, and an SSRI that helps mood does nothing for androgen levels. **Bring your full symptom list to appointments**, not just whichever symptom is loudest that week, so your doctor can see both conditions at once.

Note

Don't stop or switch a hormonal medication on your own when you're managing both conditions. Combining PCOS and PMDD treatment goals is a conversation for your prescriber, not a guess.

## What helps in the days before your period?

On the days before your period, it helps to separate what's physical from what's emotional, even roughly, before you react.

- If skin, bloating, or fatigue are loudest, treat those first with rest, food, and whatever skincare routine already works for you.

- If irritability or rejection sensitivity are loudest, **that's closer to the PMDD pattern**, and it often eases faster once you create some distance from the trigger.

- Keep decisions small today. Save anything big, a tough conversation, a purchase, quitting something, for a day outside the luteal week if you can.

Niyora is a PMS and PMDD support app. If you want somewhere to put a fight before you answer, its Moon feature lets you write what happened, see it from another angle, calm down, and draft a reply you can hold for 20 minutes.

## How do you track both?

Tracking both conditions together is more useful than tracking either alone, because it shows you which symptoms move with your cycle and which don't.

- Pick a daily tracker, like the [DRSP chart](https://niyora.com/tools/drsp-chart/), and fill it in at the same time each day.

- Log PCOS-related signs too: skin, energy, hunger, and where you are in your bleeding pattern.

- Rate mood symptoms (irritability, sadness, anxiety, rejection sensitivity) on a simple 1 to 4 scale.

- Keep this up for at least two full cycles before drawing conclusions.

- Look for the pattern. PMDD symptoms cluster hard in the final week and ease within a few days of bleeding starting. PCOS symptoms can show up anytime, with a possible bump beforehand.

If this takes a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring your two months of notes. A [PMDD specialist](https://niyora.com/care/pmdd-specialist/) or your regular doctor can read the pattern faster once it's on paper.

## What does the research say?

Research on PCOS and PMDD together is still thin, but there's useful evidence on pieces of the picture.

A 2025 systematic review of 31 randomized trials involving more than 3,000 women (Nutrition Reviews, doi.org/10.1093/nutrit/nuae043) found that vitamin B6, calcium, and zinc consistently eased the psychological symptoms of premenstrual syndrome. A small 2013 trial in Acupuncture in Medicine, with 30 women with PMDD, found a larger drop in anxiety and depression scores compared with sham acupuncture.

None of these trials screened specifically for PCOS, so it isn't clear whether the same results hold for women managing both conditions. What the evidence does support is that premenstrual mood symptoms respond to more than one kind of treatment, nutritional, hormonal, and psychological, which matters if you're already on medication for PCOS and want options that won't clash with it.

No study in this review looked directly at how PCOS changes the course or treatment of PMDD. That's the real gap, and until it closes, tracking your own pattern over two cycles stays the most reliable way to see what's happening for you.

## Questions people ask

Is PMDD caused by PCOS?

+

No. PMDD isn't caused by PCOS, and the two conditions work through different mechanisms. PCOS involves androgen and insulin levels that stay high most days of the month, while PMDD is a reaction to the normal hormone drop before a period. Having PCOS doesn't make PMDD more likely on its own, though the two can occur together.

What is the difference between PCOS and PMDD?

+

PCOS is a hormone and metabolic condition present most days of the month, with signs like irregular periods, acne, and insulin resistance. PMDD is a mood disorder tied to the final week before a period, with symptoms like irritability, low mood, or anxiety that ease once bleeding starts.

Can PCOS make PMDD symptoms worse?

+

PCOS doesn't worsen PMDD through a known hormonal mechanism, but overlapping symptoms, fatigue, mood dips, bloating, can make the luteal week feel heavier when you're managing both. Stress and premenstrual symptom severity move together in general (BJPsych, 2025), though no study has tested whether PCOS's added stress specifically worsens symptoms on a premenstrual day.

How do I know if it's PCOS or PMDD?

+

Track both for two full cycles. PMDD symptoms cluster in the final week before a period and ease within days of it starting, while PCOS symptoms can show up anytime, with a possible bump beforehand. A daily symptom chart makes that pattern far easier to see than memory alone.

Do birth control pills help both PCOS and PMDD?

+

Some combined pills can help with both. They regulate cycles and lower androgens for PCOS, and some formulations also ease depressive and overall premenstrual symptoms in trials. But not every pill that helps PCOS has strong PMDD evidence behind it, so a doctor needs to match the formulation to both goals.
