# PMDD vs depression: how to tell them apart

PMDD symptoms follow your cycle: they build in the week before your period and ease within a few days of it starting, with a clearer week after. Depression doesn't run on that schedule, it can start any time and stay. You can have both at once, and a history of depression raises the odds of a PMDD diagnosis.

PMDD

Starts in the week before your period, eases within days of it starting

Depression (MDD)

Can start or continue regardless of cycle phase

## At a glance

- PMDD is defined by timing: at least one core mood symptom in the final week before your period that eases within days of it starting (DSM-5-TR).
- Depression has no built-in cycle. Symptoms can persist most days for at least two weeks, regardless of where you are in your month.
- A past or current depression diagnosis is linked to a higher chance of a PMDD diagnosis, and the reverse connection shows up too.
- Daily charting across at least two cycles is the only reliable way to see which pattern you're in.
- SSRIs help both, but PMDD sometimes responds to dosing used only in the two weeks before your period, while depression usually needs continuous dosing.

| Compared on | PMDD | Depression (MDD) |
| --- | --- | --- |
| Timing | Starts in the week before your period, eases within days of it starting | Can start or continue regardless of cycle phase |
| Symptom-free window | Minimal symptoms in the week after your period | No required symptom-free period |
| Duration needed to diagnose | At least two cycles of daily ratings | At least two weeks of persistent symptoms |
| Core requirement | At least one mood symptom tied to the cycle | Low mood or loss of interest most of the day, nearly every day |
| Common first-line treatment | SSRIs, sometimes luteal-phase-only dosing, or certain hormonal options | SSRIs taken continuously, often alongside CBT |

## What's the difference between PMDD and depression?

PMDD and depression can look alike in the moment: low mood, irritability, trouble sleeping. The real difference is timing. PMDD symptoms build in the week before your period, peak just before bleeding starts, and ease within a few days once it does. Expert treatment guidelines describe this as a predictable, cyclic pattern that begins in the late luteal phase and clears shortly after your period starts (treatment guideline, Journal of Women's Health, 2006).

Depression doesn't follow your cycle this way. It can start on any day, last for weeks, and continue whether your period is near or not. There's no built-in symptom-free week.

Either pattern deserves real care, and noticing which one fits doesn't make the distress less real.

If you want the fuller picture of what counts as PMDD, see [PMDD, explained](https://niyora.com/pmdd/). If a persistent low mood is the pattern you recognize, read about [hormonal depression](https://niyora.com/conditions/hormonal-depression/) instead.

## Can you have PMDD and depression at the same time?

Yes, you can have both. A systematic review of studies on PMDD and depression found that nearly all of them reported a positive association between the two, including a link between a history of depression and a current PMDD diagnosis (Journal of Affective Disorders Reports, 2023). Fewer than half of those studies used the daily prospective ratings a confirmed PMDD diagnosis needs, so some of that overlap may reflect incomplete diagnoses rather than two fully separate conditions.

This connection shows up elsewhere too: a systematic review linked a PMDD diagnosis to a higher chance of depression around pregnancy and after birth, particularly postpartum depression (Archives of Women's Mental Health, 2022).

There's a biological thread as well. In one study, women with PMDD and no prior depression had a different ratio of progesterone to its calming byproduct allopregnanolone than women with a depression history (Psychoneuroendocrinology, 2006). That difference suggests the two groups may process hormone shifts differently, even when symptoms look similar.

## How do the symptoms overlap?

PMDD and depression share a real symptom list, which is part of why they get confused:

- Low or hopeless mood

- Irritability or anger

- Anxiety or feeling on edge

- Trouble concentrating

- Changes in sleep or appetite

- Low energy or fatigue

What sets them apart isn't which symptoms show up, it's whether they're tied to your cycle. PMDD requires at least one core mood symptom, mood swings or rejection sensitivity, irritability, low mood or anxiety, that shows up specifically in the week before your period. Depression doesn't need that cyclical pattern at all, it can be flat and constant across weeks.

That's also why a single snapshot, like how you feel today, can't tell you which one you're dealing with. You need the pattern over time, not one data point.

## Do I have PMDD or depression?

You can't answer this from memory alone. Most people underestimate how much their mood actually varies across the month, and both conditions involve weeks that are genuinely hard. The only reliable way to tell is to track your mood daily, across at least two cycles, and look at where the hard days fall.

A [Daily Record of Severity of Problems (DRSP) chart](https://niyora.com/tools/drsp-chart/) gives you a structured way to do this. On each day, for two cycles, you'd rate:

- Mood swings or sensitivity to rejection

- Irritability or anger

- Low mood or hopelessness

- Anxiety or tension

- Physical symptoms like bloating or breast tenderness

- How much symptoms affected your work, relationships or daily life

Afterward, you can see whether the hard days cluster in the week before your period or spread out evenly across the month.

If you want somewhere to put a hard day while it's happening, Niyora is a PMS and PMDD support app. Its Moon feature lets you write what happened in the moment, so you have real entries ready to show a doctor later, not just memory.

## How do doctors tell them apart?

A doctor will ask about your symptoms over time, not just how you feel today. For PMDD, DSM-5-TR requires daily ratings across at least two menstrual cycles showing symptoms that start in the final week before your period and ease within a few days of it starting. For depression, the requirement is different: symptoms persisting most of the day, nearly every day, for at least two weeks, regardless of your cycle.

In practice, this process doesn't always go smoothly. A scoping review of barriers to PMDD care found that many clinicians don't consistently follow diagnostic guidelines, and some people face dismissive or confusing interactions before getting an accurate diagnosis (Reproductive Sciences, 2025). That mismatch matters. Without a diagnosis that accounts for timing, treatment can target the depression without addressing the cyclical piece. Bringing your own charted data can help cut through that.

For a first step, [how to get a PMDD diagnosis](https://niyora.com/care/how-to-get-diagnosed/) walks through what to bring to that appointment. If you've been told it's one thing or the other and it hasn't fit, [finding a PMDD specialist](https://niyora.com/care/pmdd-specialist/) covers how to find someone who treats it regularly.

## How is treatment different for PMDD and depression?

Both conditions can respond to SSRIs, but the way they're used often differs. Depression guidelines call for continuous daily dosing, usually for months, as part of a broader treatment plan (expert consensus guideline, Postgraduate Medicine, 2001). PMDD can sometimes be treated with the same medications taken only in the luteal phase, the two weeks before your period, since relief doesn't require constant dosing when symptoms are cyclical (treatment guideline, Journal of Women's Health, 2006). Certain hormonal contraceptives and CBT can also help with PMDD specifically.

None of this is a choice you make alone. A doctor looks at your charted pattern, what's persisted across two cycles and what's eased on its own, before deciding what fits.

When to see a doctor

If your symptoms take a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## Questions people ask

Can you have PMDD and MDD at the same time?

+

Yes. PMDD and major depressive disorder can occur together, and a history of depression is linked to a higher chance of a PMDD diagnosis. A systematic review found most studies reported a positive association between the two, though fewer than half used the daily ratings needed to confirm PMDD (Journal of Affective Disorders Reports, 2023).

Do I have PMDD or depression?

+

The clearest clue is timing. PMDD symptoms build in the week before your period and clear within a few days of it starting, with a lighter week after. Depression doesn't follow your cycle this way. Tracking your mood daily for two cycles on a DRSP chart shows the pattern clearly.

What is PMDD depression?

+

PMDD depression is the low mood that can appear as one of PMDD's required symptoms in the week before your period, not a separate diagnosis. Unlike major depression, this low mood eases within days of your period starting and stays minimal the rest of the month.

How do symptoms of PMDD and depression overlap?

+

PMDD and depression share symptoms like low mood, irritability, trouble concentrating and changes in sleep or appetite. The difference is pattern, not symptom list. PMDD's symptoms cycle with hormone changes before your period, while depression's symptoms can show up and persist at any point in the month.

How is PMDD treatment different from depression treatment?

+

Both can respond to SSRIs, but PMDD sometimes responds to dosing only in the luteal phase, the two weeks before your period, while depression usually needs continuous daily dosing. Certain hormonal treatments and CBT can also help PMDD. A doctor decides what fits based on your charted pattern.
