# Grounding techniques: what the term means

- Grounding means using your senses (sight, touch, hearing, smell, taste) to bring your attention back to the present. - It's often used for anxiety or racing thoughts that sharpen before a period. - It doesn't change your hormones, but it can make a hard moment easier to get through.

## At a glance

- Grounding means using your senses, like sight, touch, hearing, smell and taste, to bring your attention into the present moment.
- It's commonly used for the anxiety, irritability or racing thoughts that can sharpen in the days before a period.
- A simple version is the 5-4-3-2-1 method: five things you see, four you touch, three you hear, two you smell, one you taste.
- Grounding hasn't been tested directly in trials; related mindfulness and relaxation techniques show modest benefit for premenstrual symptoms in small studies.
- It can calm an intense moment, but it doesn't replace a doctor's diagnosis or treatment plan for PMDD.

## Strength of evidence

Limited evidence

- None
- Limited
- Mixed
- Moderate
- Strong

Grounding hasn't been studied as its own treatment in published trials. The evidence comes from related mindfulness and relaxation techniques, which show modest benefit for premenstrual symptoms in small randomized trials.

## What is grounding?

Grounding is a set of techniques that bring your attention back to your body and your surroundings when anxiety or a heavy moment takes over. The idea comes from mindfulness and relaxation practice: instead of following the thought loop in your head, you name what you can sense right now.

A common version is the 5-4-3-2-1 method:

- Name five things you can see.

- Name four things you can touch.

- Name three things you can hear.

- Name two things you can smell.

- Name one thing you can taste.

Other versions use slow breathing, pressing your feet into the floor, or holding something cold. Each version does the same job. It pulls your attention out of the loop and into the present.

## How does grounding relate to PMS and PMDD?

In the days before your period, estrogen and progesterone drop, and allopregnanolone, a hormone your brain makes from progesterone, falls with them. This is a different reaction to normal hormone changes, not abnormal hormone levels (Schmidt et al., 1998 [5]; Dubey et al., 2017 [6]). For some people, the drop makes anxiety, irritability and rejection sensitivity sharper. See [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/) and [allopregnanolone](https://niyora.com/glossary/allopregnanolone/) for more on the mechanism.

Grounding doesn't change the hormones. It gives you something concrete to do while the wave passes, especially during:

- A sudden surge of anxiety in the luteal phase

- Racing thoughts after a fight you can't stop replaying

- The moment before you say something you'll regret

Niyora is a PMS and PMDD support app. If you want somewhere to put the fight before you answer, its Moon feature lets you write what happened, see it from another side, and draft a reply you can hold for 20 minutes.

In the moment

Right now: name five things you can see, four you can touch, three you can hear, two you can smell, one you can taste. Then slow your exhale to six counts before you answer anything.

## What does the research say about grounding?

Grounding itself hasn't been tested as its own treatment in the trials we found. The closest evidence comes from related mindfulness and relaxation techniques used for premenstrual symptoms.

A systematic review of meditation-based practices in medical settings found some of the strongest evidence for any condition studied was for premenstrual symptoms, though the trials were small, 958 people across 20 studies in total [1].

A separate study of mindfulness meditation taught to nursing students found it lowered premenstrual symptom severity scores [4].

Structured relaxation methods, including Benson's technique, improved premenstrual symptoms in a small randomized trial [2], though a review of the same techniques notes the evidence base stays thin and needs larger studies [3].

| Technique | What it involves | Evidence strength |
| --- | --- | --- |
| Mindfulness meditation | Noticing sensations and breath without judgment | Moderate: lower symptom scores in a systematic review of small RCTs [1]; a single uncontrolled pretest-posttest study in nursing students also found lower scores, though without a control group [4] |
| Relaxation (e.g. Benson's method) | Structured breathing and muscle relaxation | Moderate: significant improvement in a small RCT [2], though reviewers call for more data [3] |
| Grounding (5-4-3-2-1 and similar) | Naming senses to anchor attention | Limited: not tested on its own, draws on the techniques above |

What the research says

None of these trials tested grounding by name. They tested mindfulness and relaxation techniques built on the same idea as grounding, shifting attention to the body and the present moment.

## What related terms should you know?

Grounding sits inside a bigger picture of how your cycle affects mood. These related pages fill in the context:

- [PMDD (premenstrual dysphoric disorder), explained](https://niyora.com/pmdd/): the diagnosis behind the most intense version of this pattern.

- [The luteal phase](https://niyora.com/luteal-phase/): the part of the cycle where these hormone drops happen.

- [Corpus luteum](https://niyora.com/glossary/corpus-luteum/): the structure that makes the progesterone that later falls.

- [PMS and PMDD research, summarised](https://niyora.com/research/): more of the evidence behind coping tools like this one.

If what you're feeling takes a week of every month, or includes thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes. Grounding can steady a hard moment, but it isn't a diagnosis.

## Questions people ask

What does grounding mean?

+

Grounding means using your senses, like sight, touch, hearing, smell and taste, to bring your attention back to the present moment when anxiety or a heavy feeling takes over. It's a mindfulness-based technique, not a diagnosis or a treatment on its own.

How do you do a grounding exercise?

+

A common grounding exercise is the 5-4-3-2-1 method: name five things you see, four things you can touch, three you hear, two you smell, and one you taste. Slow breathing or pressing your feet into the floor work the same way.

Does grounding help PMDD anxiety?

+

Grounding can help calm a surge of anxiety or racing thoughts in the moment, including in the days before a period. It doesn't change the hormone shifts behind PMDD and isn't a substitute for a diagnosis or treatment plan from a doctor.

Is grounding backed by research?

+

Grounding itself hasn't been tested as its own treatment in published trials. Related mindfulness and relaxation techniques have shown real, modest benefit for premenstrual symptoms in small studies, which is where most of the current evidence comes from.
