# Premenstrual disorders: what it means

"Premenstrual disorders" is the umbrella term for conditions where hormone shifts before a period change your mood or body: PMS, PMDD, and premenstrual exacerbation of another condition. Each pattern starts, ends, and responds to treatment differently. Naming the right one shapes what helps.

## At a glance

- "Premenstrual disorders" covers three patterns: PMS, PMDD, and premenstrual exacerbation (PME) of an existing condition.
- All three are reactions to normal hormone shifts, not abnormal hormone levels.
- PMDD and PME diagnoses both rely on daily symptom tracking across at least two cycles, not one cycle.
- Treatments differ by pattern: SSRIs and some contraceptives for PMDD, dose changes during the luteal phase for PME.
- Low mood, irritability or anxiety that take up a week of every month are worth raising with a doctor.

## What is a premenstrual disorder?

"Premenstrual disorders" is the umbrella term for three patterns linked to the hormone shifts before a period: PMS and PMDD, which ease once bleeding starts (ACOG, 2023) [3], and premenstrual exacerbation, where an existing condition worsens before a period without fully easing once it starts.

- Premenstrual syndrome (PMS): physical or emotional symptoms in the one to two weeks before a period, common, and resolving once your period starts.

- Premenstrual dysphoric disorder (PMDD): a DSM-5-TR diagnosis needing at least five symptoms, one of them a mood symptom, confirmed with daily tracking over two cycles.

- Premenstrual exacerbation (PME): an existing condition, like depression or anxiety, that worsens before a period but stays present all month (American Journal of Obstetrics and Gynecology, 2018) [5].

PMDD, and likely PMS, come from a different reaction to normal hormone changes rather than abnormal hormone levels themselves (Schmidt et al., 1998, NEJM; Dubey et al., 2017, Molecular Psychiatry). Whether PME shares that same mechanism is still unclear [1]. For more on what drives that reaction, see [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/).

## What does the research say about premenstrual disorders?

Most research on premenstrual disorders focuses on PMDD, but premenstrual exacerbation (PME) is catching up.

Around 60% of women with an existing mood disorder report it worsening before their period, a separate pattern from PMDD itself (Current Psychiatry Reports, 2021) [1].

A 2022 review found clear evidence of premenstrual exacerbation in several conditions (Archives of Women's Mental Health, 2022) [4]:

- Psychotic disorders

- Panic disorder

- Eating disorders

- Depression

- Borderline personality disorder

Evidence was less consistent for anxiety.

The overlap between PME and PMDD's underlying biology still isn't clear. Either way, diagnosis relies on tracking symptoms daily across two cycles, rather than judging from a single week before her period (ISPMD consensus, 2016) [2].

A 2024 thematic synthesis of 17 studies and 479 women in Frontiers in Psychiatry described premenstrual disorders as intense and life changing, with women often left to manage and explain the condition largely on their own [7].

## Why does the label matter for treatment or coping?

It matters because the three patterns respond differently to the same treatments.

| Pattern | Present all month? | What tends to help |
| --- | --- | --- |
| PMS | No, only before your period | Lifestyle changes, sometimes SSRIs or certain contraceptives |
| PMDD | No, mainly the final luteal week | SSRIs (sometimes luteal-phase only), some hormonal contraceptives, CBT |
| PME | Yes, worsens before your period | Adjusting the dose of the existing treatment during the luteal phase |

Treatments that work well for PMDD often help less with PME, since the underlying condition needs its own plan, which a doctor may adjust during the luteal phase (Current Psychiatry Reports, 2021) [1].

Getting the label right matters: an SSRI dosed for PMDD is a different plan from adjusting medication for a mood disorder that flares before your period. A doctor decides which pattern fits, based on your symptom diary (Swiss interdisciplinary consensus, 2017) [6].

When to see a doctor

If low mood, irritability or anxiety take up a week of every month, see a GP or gynecologist and bring two cycles of daily notes.

## What related terms should you know?

A few related terms come up alongside "premenstrual disorders," and knowing them helps you read the research and talk to a doctor using the same words clinicians use.

- PMDD (premenstrual dysphoric disorder): the clinical diagnosis under this umbrella, with its own tracking criteria. See [PMDD explained](https://niyora.com/pmdd/).

- Luteal phase: the back half of your cycle, when all three patterns show up. See [the luteal phase](https://niyora.com/luteal-phase/).

- Corpus luteum: the structure that makes progesterone after ovulation. See [corpus luteum](https://niyora.com/glossary/corpus-luteum/).

- Allopregnanolone: a hormone byproduct linked to PMDD's mood effects. See [allopregnanolone](https://niyora.com/glossary/allopregnanolone/).

If a doctor uses one of these words and it doesn't match what you're feeling, say so and ask what it means for your particular pattern. ## Research on PME

173 studies in the library look at PME in PMS and PMDD. They were published from 1972 to 2026, and each one has a short summary of what was tested and found.

## Questions people ask

Is a premenstrual disorder the same as PMS?

+

No. Premenstrual disorder is the umbrella term, and PMS is one pattern under it, alongside PMDD and premenstrual exacerbation of another condition. PMS describes symptoms common in the one to two weeks before a period that ease once it starts (ACOG, 2023).

Can a premenstrual disorder be diagnosed from a single month?

+

No. Diagnosing a premenstrual disorder needs symptom tracking across at least two menstrual cycles, since one week of symptoms could have other causes (ISPMD consensus, 2016). Daily ratings show whether your symptoms actually track the cycle, which is what separates PMDD or PME from a one-off stretch of symptoms.

What's the difference between PMDD and premenstrual exacerbation?

+

PMDD is a stand-alone diagnosis that appears only in the week before your period and eases within days of it starting. Premenstrual exacerbation is an existing condition, like depression, that's present all month and worsens in that same week (American Journal of Obstetrics and Gynecology, 2018).

How common is it to have a mood disorder alongside a premenstrual disorder?

+

Very common. A 2026 meta-analysis of 3,646 women found consistently high rates of PMDD or PMS alongside mood disorders like depression and bipolar disorder, with exact figures varying depending on which diagnosis came first and whether both were tracked together (British Journal of Psychiatry, 2026).
