# Prostaglandins: what it means

Prostaglandins are hormone-like compounds your body makes that cause the uterus to contract and drive pain and inflammation. They're closely tied to period cramps and some physical PMS symptoms, though less directly to PMDD's mood symptoms. NSAIDs work partly by lowering them.

## At a glance

- Prostaglandins are hormone-like compounds made from fatty acids that make the uterus contract and drive inflammation.
- They're tied mainly to physical symptoms like cramps, breast tenderness and headaches, not directly to PMDD's mood symptoms.
- A small case-control study found women with PMS had a mixed prostaglandin pattern compared with women without it ( [3]).
- NSAIDs like naproxen lower prostaglandin levels and ease period pain at the same time ( [4]).

## What are prostaglandins?

Prostaglandins are hormone-like compounds your body makes from fatty acids in cell membranes. Unlike estrogen or progesterone, they don't travel far through the bloodstream. They act locally, right where they're made, often within minutes. They're made on demand rather than stored, so levels can rise quickly in the days before a period.

In the body, prostaglandins:

- Make the uterus contract, which causes menstrual cramps

- Trigger inflammation and swelling

- Sensitize nerves, so pain feels sharper

- Affect blood flow to tissues

The stronger the prostaglandin response in the uterine lining, the more painful the cramping tends to be. That's why prostaglandins come up often in conversations about period pain, and sometimes PMS.

## How are prostaglandins linked to PMS and PMDD?

PMS covers physical and emotional symptoms in the one to two weeks before a period that ease once bleeding starts. Prostaglandins are tied mainly to the physical side: cramps, breast tenderness, bloating and headaches.

A case-control study tracked 20 women with PMS across three menstrual cycles and compared them with 12 women without PMS, who were measured over one cycle. The PMS group had lower PGF2 alpha in the late luteal phase, but higher PGE, PGE2 and TXB2 at several other points in the cycle ( [3]). That mixed pattern suggests a disturbance in how the body handles prostaglandins, rather than a simple excess.

[PMDD](https://niyora.com/pmdd/) is a separate diagnosis built around mood symptoms: mood swings, rejection sensitivity, irritability or low mood, in the final week before a period. Prostaglandins haven't been shown to drive that mood pattern. Current thinking, covered in [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/), points instead to an unusual brain sensitivity to normal swings in [allopregnanolone](https://niyora.com/glossary/allopregnanolone/).

## What does the research say?

Research on prostaglandins and the menstrual cycle is older and mostly small. It's still useful for understanding why cramps and premenstrual pain happen.

| Study focus | What it measured | What it found |
| --- | --- | --- |
| Dysmenorrhea vs. premenstrual pain ( [2]) | Vasopressin and PGF2 alpha metabolites in blood | PG metabolites rose during menstrual cramps but not during premenstrual pain, which tracked with vasopressin instead |
| Menstrual blood in dysmenorrhea ( [4]) | PGE1 and PGF2 alpha in menstrual fluid, before and after naproxen | Levels ran 3 to 4 times higher in women with dysmenorrhea; naproxen brought levels and pain down together |
| PMS vs. no PMS ( [3]) | PGF2 alpha, PGE, PGE2, TXB2 (PMS group: three cycles; controls: one cycle) | Mixed pattern: PGF2 alpha lower, several other prostaglandins higher, in the PMS group |

Together, these studies point to prostaglandins as a real part of menstrual cramping, and possibly PMS's physical symptoms, but the picture isn't uniform even within small studies.

## Why do prostaglandins matter for treatment and coping?

Because prostaglandins drive cramping, **NSAIDs** like ibuprofen and naproxen work partly by lowering them. One study found naproxen reduced both PGE1 and PGF2 alpha levels in menstrual blood while pain eased at the same time ( [4]).

For PMDD's mood symptoms, guidelines point to different tools:

- SSRIs, sometimes taken only in the luteal phase

- Certain hormonal contraceptives

- CBT

A doctor decides what fits your pattern. Early research also looked at gamma-linolenic acid, found in evening primrose oil, as a way to support the body's own prostaglandin balance in PMS ( [1]). That line of research is old and small, so current guidelines don't recommend evening primrose oil or GLA for PMS.

A few related terms worth knowing:

- [Corpus luteum](https://niyora.com/glossary/corpus-luteum/): the structure that makes progesterone after ovulation

- [The luteal phase](https://niyora.com/luteal-phase/): where prostaglandin and hormone shifts overlap

- [Menstrual cycle hormones, phase by phase](https://niyora.com/cycle/menstrual-cycle-hormones/): for the fuller picture

None of this changes what helps right now, when cramps show up or irritability rises at an inconvenient time. Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what's happening, see it from another angle, and calm down before you respond to anyone.

## Questions people ask

Do prostaglandins cause PMDD?

+

Prostaglandins aren't the main driver of PMDD. PMDD's mood symptoms are thought to come from an unusual brain sensitivity to normal swings in allopregnanolone, not from high or low hormone levels (see what causes PMDD). Prostaglandins are more closely tied to the physical side of PMS and period pain, like cramps and breast tenderness, than to PMDD's mood criteria.

Why do prostaglandins cause period cramps?

+

Prostaglandins made in the uterine lining cause the muscle to contract as it sheds. Higher levels, especially PGF2 alpha, are linked to stronger cramps. One study found levels 3 to 4 times higher in women with dysmenorrhea than in women without it, and naproxen lowered both levels and pain ( [4]).

Can NSAIDs lower prostaglandins for period pain?

+

Yes. NSAIDs like ibuprofen and naproxen reduce the prostaglandins that drive uterine contractions and pain. A study on naproxen found it lowered PGE1 and PGF2 alpha in menstrual blood while pain eased at the same time ( [4]). A doctor can advise on what's right for you.

Are prostaglandins linked to PMS as well as period pain?

+

Yes. A small study compared 20 women with PMS to 12 without and found a mixed prostaglandin pattern: lower PGF2 alpha in the late luteal phase, but higher PGE, PGE2 and TXB2 at other points in the cycle ( [3]). The pattern wasn't a simple high or low overall.
