# Why do antidepressants work differently for PMDD than for general depression?

A theoretical model suggests PMDD responds faster to certain antidepressants (SSRIs) because it involves different brain mechanisms than depression. This framework has not yet been tested.

 Early or limited evidence

Women with depression and PMDD (theoretical model)

## What they did

Theoretical review proposing an explanation for how antidepressant medicines (SSRIs) work in depression and PMDD.

## What they found

Major depression may involve low dopamine from overly active 5-HT2 receptors. Antidepressants work by reducing these receptors, increasing dopamine. Female hormones may activate different 5-HT2 receptors causing atypical depression and premenstrual dysphoria. In PMDD, serotonin reuptake inhibitors may work through immediate serotonin increase rather than delayed receptor changes, explaining faster response. The model predicts 5-HT2 antagonists would work fast in melancholic depression and could speed serotonin reuptake inhibitor effects, but would interfere in atypical depression and premenstrual dysphoria.

## What it means for you

PMDD and depression may involve different brain mechanisms, which could explain why PMDD responds faster to certain medications. Main limit: theoretical model, not yet tested.
