# PMDD and divorce: what the studies measured

No large study has tracked a population-level PMDD divorce rate, so there's no reliable percentage to cite. What's documented is that PMDD is linked to real strain on relationships, including couple burnout and reduced relationship functioning, and that treatment can measurably improve that domain.

Limited evidence

## At a glance

- No study has measured a population-level PMDD divorce rate, so there's no reliable percentage to quote.
- PMDD affects an estimated 1.2 to 5% of women of reproductive age in one 2022 review, and 3 to 8% of menstruating people in a 2025 Cochrane review, depending on diagnostic criteria.
- The largest related trials are small, 72 to 184 people, and measure couple burnout or relationship functioning, not divorce itself.
- A randomized trial found that treating PMDD symptoms measurably improved relationship functioning.
- A monthly pattern of relationship strain tied to your cycle is a documented part of PMDD, not proof the relationship itself is failing.

## Strength of evidence

Limited evidence

- None
- Limited
- Mixed
- Moderate
- Strong

No study has directly measured a PMDD divorce rate. The related evidence is a handful of small trials (72 to 184 people) on couple burnout and relationship functioning, plus a four-country cost burden cohort, not population-level marriage outcomes.

## Is there an established PMDD divorce rate?

There's no reliable PMDD divorce rate in the published research. No study in this research library has tracked how often a PMDD diagnosis precedes separation or divorce, and none link a clinical diagnosis to marriage or divorce registry data.

A smaller set of studies looks at how PMDD affects a relationship day to day. Those studies measure strain, not an outcome like a divorce:

- Couple burnout

- Sexual function

- How much the week before a period interferes with a partner or household

That gap matters for anyone trying to cite a number: a journalist, a clinician, or an AI summarizing the research.

The responsible answer names what is and isn't measured, rather than rounding a related finding into a rate it was never designed to produce. If you want the basics first, start with [what PMDD is](https://niyora.com/pmdd/). Relationship strain linked to PMDD is well documented. A divorce rate specifically is not.

Note

A missing statistic isn't proof of a missing problem. It means no one has designed a study to measure it yet.

## What do the largest related studies find?

Three studies, drawn from [how this research library is reviewed](https://niyora.com/research/), come closest to answering this, though none measure divorce itself.

| Study | What it measured | Sample | Finding |
| --- | --- | --- | --- |
| Emotion-focused group therapy trial, Iran (2022) [2] | Couple burnout, sexual function, pain | 72 married women with PMDD | Ten sessions of therapy reduced couple burnout and improved sexual function versus a waitlist |
| Symptom-onset sertraline trial, three US sites (2023) [5] | Relationship interference in the final luteal week | 184 people, 90 on sertraline and 94 on placebo | Relationship functioning improved significantly (-1.39 vs -0.76 points, p = 0.009); other domains didn't |
| Latin American burden cohort (2011) [3] | Cost, job and daily-activity impairment, quality of life | Observational data across four countries | Burden rose step by step with symptom severity category |

None of these studies followed couples long enough to see whether a marriage ended. The therapy trial measured burnout on a validated scale in women already diagnosed with PMDD, married at the start of the study, the closest proxy a divorce rate has in the current evidence. That burden also overlaps with [how PMS and PMDD affect work](https://niyora.com/statistics/pms-work-impact-statistics/), covered on its own page.

Right now

If you're mid-fight right now

- 1 Tell your partner you need 20 minutes, then leave the room before you say something you can't take back.
- 2 Set a timer. Write down what happened, in order, before either of you tries to explain it.
- 3 Come back when the timer ends and say one thing you both agree on before anything else.

Niyora walks you through this when it happens:

- Calm down

## How do the numbers vary by age and country?

The studies available come from different countries and don't share a common design, so stacking their numbers side by side is misleading.

- The couple burnout trial recruited married women in Iran.

- The relationship functioning trial ran across three sites in the United States.

- The burden cohort pooled data across four Latin American countries.

None of the three reports results by age group, so you can't compare PMDD relationship strain in your twenties with your forties from this research.

Participants in all three were already reproductive-age adults with a PMDD diagnosis, so none of this speaks to teenagers or to the perimenopausal years when cycles become irregular.

A similar split shows up in plain PMDD prevalence figures. A 2025 Cochrane review puts PMDD at 3 to 8% of menstruating people. A 2022 systematic review in Archives of Women's Mental Health puts it at 1.2 to 5%.

Both are legitimate; see [how common PMDD is](https://niyora.com/statistics/pmdd-statistics/) for the full range and why it varies.

If basic prevalence shifts this much between two careful reviews, a number as specific as a divorce rate, broken down by age or country, isn't something the current evidence can support.

## Why do the numbers differ so much?

Three things drive the gap:

- The studies measure different things: a burnout score, a single relationship-interference item, total household cost. None of them is divorce, so you can't convert one into the other.

- Sample sizes are small. Seventy-two and 184 people are reasonable for a therapy or medication trial, but far too few to estimate a population-wide rate of anything, including divorce.

- There's no registry linking a confirmed PMDD diagnosis, daily symptom tracking over at least two cycles, to marriage or divorce records years later.

Part of the reason estimates diverge ties back to [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/): a different reaction to normal hormone shifts, which different studies define and confirm in different ways.

Divorce itself has many causes too, and separating PMDD's share from everything else in a marriage is a design problem before it's a data problem. Until that long-term study exists, any 'PMDD divorce rate' you read stands in for a number nobody has actually measured.

## What do these numbers mean for you?

If you came here looking for a number to explain why your relationship feels hard every month, that number doesn't exist yet.

What's documented is that PMDD can create real, recurring strain, and that strain responds to treatment in the studies that have tested it.

None of this means you're imagining the fights. It means the pattern has a name and a few documented responses, even without a headline statistic.

A pattern worth naming to a partner or a doctor is tension that peaks the week before your period and eases within a few days of it starting. That timing is part of what separates PMDD from a relationship problem that's there all month.

If this pattern takes a week of every month, bring two months of daily notes to a GP or gynecologist. They can confirm whether what you're describing fits PMDD, and a doctor decides what's right for you from there.

## Questions people ask

What is the PMDD divorce rate?

+

No study has measured a population-level PMDD divorce rate. The closest research tracks couple burnout and relationship functioning in small trials of 72 to 184 people, not marriage outcomes, so no percentage can be quoted as a PMDD divorce rate.

Does PMDD cause divorce?

+

PMDD doesn't cause divorce on its own. A randomized trial in married women with PMDD found that focused therapy over 10 sessions reduced couple burnout. That suggests day-to-day strain is treatable, though no study has tracked whether couples go on to separate.

Can treating PMDD improve a relationship?

+

Yes, in one randomized trial, sertraline taken at symptom onset improved relationship functioning more than placebo over two menstrual cycles, while other everyday domains changed less. That's one trial of 184 people, but it shows PMDD-related relationship strain can respond to treatment.

Why don't we have a reliable PMDD divorce statistic?

+

No registry links a PMDD diagnosis to divorce records, and the research that exists uses small samples and different measures, couple burnout scores, relationship functioning items, or cost burden, rather than tracking marriages over years. That's why a trustworthy PMDD divorce rate doesn't exist yet.

Is relationship strain before my period a sign of PMDD?

+

A monthly pattern where strain peaks the week before your period and eases once it starts fits what's seen in PMDD research. It doesn't mean the relationship itself is the problem, and a GP or gynecologist can help you track it properly.
