# PMDD hunger: what it feels like and what helps

PMDD hunger usually shows up as intense, hard-to-satisfy cravings, or less often, a sudden loss of appetite, in the final week before your period. It happens because falling estrogen and progesterone affect serotonin and blood sugar, and PMDD brains react more strongly to that shift than most. It tends to ease within a few days of your period starting.

Limited evidence

## At a glance

- PMDD hunger can mean intense cravings, eating past full, or sometimes losing interest in food entirely, mainly in the week before your period.
- It's driven by the same hormone drop that affects everyone premenstrually. PMDD just means reacting more strongly to that normal shift.
- Appetite change is one of the 11 DSM-5 symptoms used to diagnose PMDD, so it rarely shows up alone.
- Eating protein or fat before a craving peaks, and drinking water first, both help blunt sudden hunger in the moment.
- If hunger and the symptoms around it repeat every month for about a week, track it daily for two cycles and bring the notes to a doctor.

## Strength of evidence

Limited evidence

- None
- Limited
- Mixed
- Moderate
- Strong

Direct research on PMDD hunger specifically is thin. Most evidence comes from broader PMS/PMDD treatment trials where food cravings or appetite change is listed as one symptom among many, not the primary outcome. Vitamin D showed no significant effect on cravings in a meta-analysis [2].

## What does PMDD hunger feel like?

PMDD hunger often doesn't feel like ordinary hunger. It can arrive suddenly, feel bottomless, and stay even after a full meal. For some people, it's the opposite: appetite disappears and eating feels like a chore.

Both count as the same DSM-5 symptom, a **marked change in appetite**, overeating, or specific food cravings, usually for carbohydrates or salty foods. See [PMDD symptoms: the 11 DSM-5 symptoms, explained](https://niyora.com/symptoms/pmdd-symptoms/) for the full list.

What people describe:

- Hunger that feels urgent, almost like panic, not just 'I should eat'

- Cravings for one specific food, often bread, chips, or chocolate

- Eating fast, then still feeling unsatisfied

- No interest in food at all, or forgetting to eat

- Hunger arriving at the same time as low mood, rage, or tears

## Why does PMDD cause hunger?

Estrogen and progesterone both drop sharply in the days before a period. That drop affects serotonin, the brain chemical that helps regulate mood and appetite, and it can disrupt blood sugar regulation too.

People with PMDD react more strongly to that normal hormone shift, not to abnormal hormone levels themselves (Schmidt et al., 1998; Dubey et al., 2017). That's why an identical hormone change can leave one person untouched and another ravenous by 4pm or in tears over nothing. The same shift behind mood symptoms likely drives appetite ones too.

A 2021 randomized trial on hypnotherapy for PMDD lists food cravings and binge eating among the symptoms women reported, alongside mood changes, tiredness, and trouble sleeping [4]. That fits with appetite change being one symptom in a cluster, not a separate problem.

Right now

When the hunger hits right now

- 1 Eat something with protein or fat first, like eggs or nuts, rather than reaching straight for sugar.
- 2 Drink a full glass of water. Thirst can show up as hunger this week.
- 3 Set a timer for 20 minutes before deciding whether to eat more, so your body has time to register what you've had.
- 4 If shame or rage shows up with the hunger, write down what happened before you react to it.
- 5 Keep food in reach that you don't have to negotiate with yourself about, so eating isn't a fight.

## How is it different from PMS hunger?

PMS can include hunger too, but PMDD hunger tends to be more intense and comes with more disruption to your day.

| | PMS hunger | PMDD hunger |
| --- | --- | --- |
| Timing | One to two weeks before a period | Mainly the final week before a period |
| Intensity | Noticeable, but manageable | Can feel urgent or hard to control |
| What else shows up | Mild mood or body changes | At least one mood symptom plus four more, enough to disrupt daily life |
| Eases by | A few days into the period | Within a few days of bleeding starting, minimal the week after |

If hunger alone is the main thing you notice, [hunger a week before your period](https://niyora.com/symptoms/hunger-a-week-before-period/) walks through when that's PMS and when it's worth a closer look. For the hormonal reasons behind it, see [why am I so hungry before my period](https://niyora.com/why/am-i-so-hungry-before-my-period/).

## When in your cycle is it worst?

PMDD hunger tends to build through the luteal phase, the two weeks after ovulation, and peak in the final week before your period. That's when estrogen and progesterone fall furthest and fastest.

For PMDD, the diagnosis itself depends on that timing: symptoms need to show up mainly in the week before bleeding and ease within a few days of it starting, staying minimal the week after. If your hunger doesn't follow that pattern, it may have a different cause worth asking about.

[Hunger in the luteal phase: why it happens](https://niyora.com/cycle/luteal-phase-hunger/) maps the day-by-day pattern if you want to check it against your own cycle.

## What can you do when it hits?

The urge to eat everything in sight, or to not eat at all, tends to pass more easily when you work with it instead of fighting it with willpower alone.

What tends to help:

- Eat something with protein or fat before the craving peaks, so blood sugar doesn't fall further

- Keep easy, filling food within reach instead of only highly processed snacks

- Drink water first, since dehydration can mimic hunger

- If you're not eating at all, set a quiet reminder rather than waiting to feel hungry

If the pull is toward eating past the point of comfort, [the urge to binge before your period](https://niyora.com/moments/urge-to-binge/) covers that specific feeling in more detail.

Niyora is a PMS and PMDD support app. If what's underneath the hunger is a fight you can't stop replaying, its Moon feature lets you write down what happened, see it from another angle, and draft a reply you can hold onto for 20 minutes instead of sending something you don't mean.

## Which PMDD treatments help hunger?

No treatment is approved specifically for PMDD hunger. Appetite is one symptom among several, and most evidence looks at whether a treatment reduces PMDD or PMS overall, not hunger on its own.

| Option | What it's for | Evidence |
| --- | --- | --- |
| SSRIs (like fluoxetine), luteal-phase or daily, prescribed by a doctor | Mood and physical PMDD symptoms overall | Outperformed placebo for mood and physical symptoms including bloating in randomized trials [5] [6] |
| CBT | The thinking and behavior patterns around symptoms | Recommended alongside medication in treatment reviews [3] |
| Vitamin D supplementation | Overall PMS severity | Reduced PMS severity and physical symptoms in a meta-analysis, with no significant effect on food cravings specifically [2] |
| Complex carbohydrate drink, calcium, magnesium, vitamin B6 | Nonpharmacologic support for PMS/PMDD | Some evidence for efficacy, per a treatment review [3] |
| Vitex agnus-castus (chasteberry) | General PMS symptoms | Outperformed placebo in most PMS trials reviewed, not tested specifically for hunger [1] |

A doctor decides what's right for you, based on symptom severity and what else is going on. Coping strategies for hunger in the moment can get you through, but they don't treat PMDD itself.

## How do you track it for your doctor?

PMDD is confirmed with daily symptom tracking over at least two cycles, not from memory of one premenstrual week.

- Each day, note your cycle day, whether hunger showed up, and how strong it was on a 1 to 10 scale.

- Write down what else was happening: mood, sleep, energy, cravings for a specific food.

- Mark the day your period starts and watch whether hunger eases within a few days.

- Repeat for a second cycle to see if the pattern holds.

Bring both months of notes to the appointment. If hunger and the symptoms around it take up a week of every month, or you're having thoughts of hurting yourself, see a GP or gynecologist and bring those notes with you.

## Questions people ask

Is PMDD hunger the same as PMS hunger?

+

PMDD hunger and PMS hunger share the same hormonal trigger, but PMDD hunger tends to be more intense and shows up with at least one mood symptom plus four others, disrupting daily life in the final week before a period. PMS hunger is usually milder and doesn't come with that level of disruption.

Why do I have no appetite before my period with PMDD?

+

PMDD can cause loss of appetite as well as increased hunger, since the DSM-5 criteria count any marked change in appetite, in either direction, as the same symptom. The hormone shift behind it is the same drop in estrogen and progesterone driving PMDD's other symptoms that week.

Can PMDD hunger cause weight gain?

+

PMDD hunger itself doesn't cause weight gain, though eating more during the week it happens can add up over months. Bloating and water retention, common with PMDD, often make that week feel heavier on the scale even without real weight gain.

Does PMDD hunger go away after your period starts?

+

Yes. For PMDD specifically, symptoms including hunger are expected to ease within a few days of a period starting and stay minimal the week after. If hunger or other symptoms carry on past that window, it's worth tracking and mentioning to a doctor.

What helps PMDD hunger right now?

+

Eating something with protein or fat before a craving peaks, drinking water first, and keeping filling food within easy reach all help blunt PMDD hunger in the moment. Restricting food during this week tends to make the cravings stronger, so working with the hunger usually works better than fighting it.
