# Saffron for PMDD: does it work?

One 2020 trial found saffron worked about as well as fluoxetine (20mg twice daily) for PMDD symptoms, with fewer side effects, but it's the only trial testing saffron specifically in PMDD. Dosing in that trial was 15mg of saffron extract twice daily during the luteal phase, for two cycles. Side effects were mild, mostly nausea or drowsiness.

## At a glance

- A 2020 randomized trial of 120 women found saffron (15mg twice daily) worked about as well as low-dose fluoxetine for PMDD symptoms, with fewer side effects.
- The dose used in the trial was taken during the luteal phase only, for roughly two weeks before the period, across two cycles.
- Most other saffron research covers PMS or depression, not PMDD specifically, so this rests on one small trial rather than a broad evidence base.
- Side effects are usually mild: nausea, dry mouth, drowsiness or dizziness.
- Saffron hasn't been tested alongside SSRIs or other PMDD treatments, so combining them hasn't been studied.

One small randomized trial found saffron worked about as well as low-dose fluoxetine for PMDD, with fewer side effects, but it's the only PMDD-specific trial so far. The rest of the evidence is on PMS, not PMDD, so reviewers call saffron promising but still unproven at scale for PMDD.

## Does saffron work for PMDD?

Saffron shows real promise for PMDD, but it rests on surprisingly little research.

The one trial testing saffron specifically for PMDD found it worked about as well as the SSRI fluoxetine (20mg twice daily), with fewer side effects (Advanced Biomedical Research, 2020). That result comes from a single study of 120 women over two cycles, not a body of repeated findings.

Most of what we know about saffron and mood actually comes from PMS and depression trials, not PMDD specifically. A systematic review of saffron trials on psychological outcomes found consistent positive effects, but called for larger trials before drawing firm conclusions (Journal of Integrative Medicine, 2015).

Wider reviews of saffron's effects on the brain also describe it as promising and well tolerated, though not yet proven at scale (Avicenna Journal of Phytomedicine, 2022). **Saffron looks hopeful for PMDD**, backed by one solid trial, but it hasn't been confirmed the way SSRIs or CBT have.

## What does the research say?

The strongest data point is a 2020 randomized trial of 120 women diagnosed with PMDD, published in Advanced Biomedical Research. Over two luteal-phase cycles, researchers compared saffron, fluoxetine and placebo.

| Group | Dose | Result |
| --- | --- | --- |
| Saffron | 15mg, twice daily | Clear drop in symptom severity scores |
| Fluoxetine | 20mg, twice daily | Similar-sized drop in symptom scores |
| Placebo | None | Smaller improvement in DRSP symptom-severity scores than the active treatments (P=0.027 between-group comparison); depression scores on the Hamilton scale didn't differ significantly across the three groups (P>0.05) |

Both saffron and fluoxetine produced similar drops in daily symptom (DRSP) scores; on the Hamilton depression measure, the three groups didn't differ significantly (P>0.05). Fluoxetine caused more side effects along the way, which is why researchers concluded saffron worked about as well with less burden.

Outside that one trial, the evidence is mostly about PMS, not PMDD. A meta-analysis of PMS trials found saffron reduced symptom severity by a solid margin, with a standardized mean difference of -0.64 (Korean Journal of Family Medicine, 2026).

A systematic review of saffron's effects on the female reproductive system reached a similar conclusion for PMS and related cycle symptoms (Phytotherapy Research, 2024). None of that is specific to PMDD, which is a distinct, more severe diagnosis, so it only tells us saffron likely helps mood tied to the cycle in general, not that it's confirmed for PMDD.

What the research says

Saffron matched fluoxetine (20mg twice daily) for PMDD symptoms in this trial, with fewer side effects. It's the only trial of its kind so far, so this is promising, not proven.

## How do you take saffron for PMDD?

In the PMDD trial, participants took 15mg of standardized saffron extract twice daily, starting around two weeks before their expected period and continuing through that window, for two full cycles (Advanced Biomedical Research, 2020). That's the same two-weeks-on pattern used for luteal-phase SSRIs.

A few practical points if you're considering it:

- Trials use standardized extract capsules, not raw saffron threads or spice from the kitchen.

- Dosing matched the luteal phase only, not every day of the cycle.

- No higher dose has been shown safer or more effective than what was studied.

For a fuller breakdown of amounts, timing and what to look for on a label, see [saffron dosage: how much and when](https://niyora.com/treatments/saffron-dosage/).

## How long does it take to work?

Researchers in the PMDD trial measured results after two full luteal-phase courses, each two weeks long, so about two menstrual cycles in total (Advanced Biomedical Research, 2020). That's how long they waited before comparing scores, not proof that nothing happens sooner.

Saffron's broader mood research, including depression trials, usually measures change over several weeks of daily use, not days (Journal of Integrative Medicine, 2015). Two cycles is a reasonable minimum before deciding whether it's working for you.

If hard feelings show up before you've finished those two cycles, that doesn't mean it's failing. It means you're still in the window the research actually tested.

For a week-by-week sense of what to expect, see [how long does saffron take to work](https://niyora.com/treatments/how-long-does-saffron-take-to-work/).

## What are the side effects?

Saffron's side effects are usually mild. In the PMDD trial, saffron caused significantly fewer side effects than fluoxetine (P=0.01) (Advanced Biomedical Research, 2020).

Broader reviews of saffron for mood and related conditions describe it as generally safe and well tolerated at the doses studied (Avicenna Journal of Phytomedicine, 2022).

Reported side effects include:

- Nausea

- Dry mouth

- Drowsiness or dizziness

- Changes in appetite

These tend to ease on their own as your body adjusts. None of the trials behind this page reported serious harm at the doses used, though research in pregnancy, breastfeeding and in people under 18 is too thin to draw on.

For the fuller list, including rarer reactions and when to stop, see [saffron side effects, including mood](https://niyora.com/treatments/saffron-side-effects/).

## Who should avoid it?

Saffron looks low-risk for most people in the trials so far, but a few groups need extra caution.

- If you're pregnant or trying to conceive, since saffron hasn't been tested for mood in pregnancy, and some forms affect the uterus.

- If you're already taking an SSRI or another medication that affects serotonin, since no trial has tested combining them.

- If you have a known allergy to saffron, iris or related plants.

Saffron still looks low-risk for most people. The caution above is for groups the research hasn't covered, so a quick conversation first is worth it if you fall into one of them.

When to see a doctor

If PMDD symptoms take up a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## What else helps alongside it?

Saffron doesn't have to stand alone. The treatments with the most consistent evidence for PMDD are SSRIs (sometimes taken only in the luteal phase), certain hormonal contraceptives, and CBT, and a doctor can help you weigh which fits (see [PMDD treatment: what helps](https://niyora.com/treatments/pmdd-treatment/)).

## Questions people ask

Does saffron work as well as antidepressants for PMDD?

+

In one 2020 trial of 120 women, saffron (15mg twice daily) worked about as well as fluoxetine (20mg twice daily) for PMDD symptoms, with fewer side effects. That's a single study, though, so it hasn't been confirmed the way SSRIs have been across many trials.

How much saffron should I take for PMDD?

+

The PMDD trial used 15mg of saffron extract twice daily, started about two weeks before the period and continued through two luteal-phase windows. No higher dose has been shown safer or more effective, so more isn't automatically better.

Can saffron help PMDD and ADHD together?

+

No trial has tested saffron for PMDD and ADHD together. Saffron's mood effects in PMDD come from one small trial, and ADHD symptoms that worsen before your period (premenstrual exacerbation) need their own plan with your prescriber.

What's the best saffron supplement for PMDD?

+

The trial that worked used a standardized saffron extract, 15mg twice daily, not raw saffron threads or unregulated blends. Look for a product that states its crocin or safranal content, since that's what's actually been tested for PMDD.

Are there side effects from taking saffron for PMDD?

+

Saffron's side effects are usually mild: nausea, dry mouth, drowsiness or dizziness. In the PMDD trial, saffron caused fewer side effects than fluoxetine, and reviews call it generally safe and tolerable at the doses studied.
