# SSRIs for PMDD: how they work, and what to expect

SSRIs are the best-studied medicine for PMDD and the first treatment most doctors offer. Unlike in depression, they often work within a few days, so many people take them only in the two weeks before their period.

## At a glance

- SSRIs such as fluoxetine, sertraline, paroxetine and escitalopram reduce PMDD symptoms more than placebo across dozens of trials.
- They tend to act within **1 to 2 days** in PMDD, which is why luteal-phase dosing (only from ovulation to your period) is an option.
- Irritability, mood swings and anxiety respond best. Physical symptoms like bloating improve less.
- The most common side effects are nausea, tiredness and lower sex drive. Most are mild and fade.
- They do not fix the hard moment itself. Pairing medicine with a way to handle the moment works better than either alone.

A Cochrane review of 31 randomised trials found SSRIs reduce premenstrual symptoms compared with placebo, whether taken all month or only in the luteal phase.

## Why do SSRIs help PMDD?

PMDD isn't caused by abnormal hormone levels. In people with PMDD, estrogen and progesterone are usually in the normal range. The difference is how the brain responds to the normal rise and fall of estrogen and progesterone after ovulation.

Serotonin sits in the middle of that response. Progesterone's breakdown product, allopregnanolone, acts on the same calming brain receptors that serotonin helps regulate. When that system is extra sensitive, the drop before your period lands as rage, tears or dread. SSRIs keep more serotonin available, which steadies the system through the shift.

What the research says

One leading explanation is that SSRIs raise allopregnanolone in the brain. That would explain why they act in days in PMDD rather than weeks.

## How fast do SSRIs work for PMDD?

In depression, SSRIs take four to six weeks. In PMDD they often work within the first one or two days of a cycle. That speed is what makes **luteal-phase dosing** possible: you take the medicine from around ovulation until your period starts, then stop.

There are three common ways to take them:

- **Every day**, all month. Best studied, and simplest if your cycle is irregular.

- **Luteal phase only**, from about day 14 to the start of your period.

- **Symptom-onset**, starting the day symptoms begin. Less studied, and works for some people.

Right now

When it hits before the medicine has helped

- 1 Name it to yourself: this is the luteal drop, and it has an end date.
- 2 Breathe out longer than you breathe in, five times.
- 3 Don't send anything for twenty minutes. Write it instead.
- 4 Decide what you actually need from the other person, in one sentence.

## Which SSRIs are used for PMDD?

In the US, fluoxetine, sertraline and paroxetine CR are approved for PMDD. Escitalopram and citalopram have good trial data too. No single SSRI clearly beats the others, so the choice usually comes down to side effects and what you have tried before. Doses are often lower than for depression.

If one SSRI does not help after two or three cycles, another often does. Venlafaxine, an SNRI, is a common next step.

## What are the side effects?

The most common are nausea, tiredness, trouble sleeping, sweating and lower sex drive. They are usually mild and often fade within a couple of weeks. Luteal-phase dosing can mean fewer side effects overall, because you take the medicine for half the month.

Stopping suddenly after daily use can cause dizziness and irritability for a few days, so talk to your doctor before you stop.

## Do SSRIs replace other support?

No. Medicine turns the volume down. It does not tell you what to say when your partner walks out mid-sentence, or how to answer a two-line email from your manager. Cognitive behavioural therapy has its own evidence in PMDD, and people often do best with both.

That is the gap Niyora was built for. When the hard moment still comes, Moon helps you see it from another angle, settle your body, and choose your next words.

When to see a doctor

SSRIs need a prescription. Bring two cycles of daily symptom notes to the appointment, because PMDD is diagnosed from symptoms tracked across cycles.

## Research on SSRIs

513 studies in the library look at SSRIs in PMS and PMDD. They were published from 1971 to 2026, and each one is summarised in plain English.

## Questions people ask

Can I take SSRIs only before my period?

+

Yes. Because SSRIs act within days in PMDD, many people take them only in the luteal phase, from about ovulation until their period starts. Trials show this works for most people, though daily dosing is simpler if your cycle is irregular.

What is the best SSRI for PMDD?

+

There is no single best one. Fluoxetine, sertraline and paroxetine CR are approved for PMDD in the US, and escitalopram also has strong trial data. The right choice depends on side effects and your history, so it is a decision to make with your doctor.

Will SSRIs help with PMS too?

+

SSRIs are mainly studied and used for PMDD and severe PMS. For milder PMS, lifestyle changes, calcium and cognitive approaches are usually tried first.

How long until I know if it works?

+

Give it two to three cycles. Because PMDD responds quickly, you should notice a difference within the first few luteal phases. If you don't, ask about a different SSRI or dosing pattern.
