Does rimegepant prevent migraines around your period?

Rimegepant prevented perimenstrual migraines in 85% of cycles with no side effects in this small study of 5 women, but we need larger studies to be confident in these results.

Early or limited evidence 5 women aged 22-42 with episodic migraine and regular menstrual cycles

What they did

Case series of 5 women aged 22-42 with episodic migraine and regular menstrual cycles. They took rimegepant 75 mg every other day starting 2 days before menstruation and continuing 2 days after menstruation. Patients kept daily headache diaries and were followed for 3-4 cycles.

What they found

Rimegepant prevented perimenstrual attacks in 17 of 20 cycles (85%) with no treatment-related side effects. However, only 1 patient (20%) had sustained reduction in overall monthly migraine days. Three patients (60%) experienced delayed migraine onset 3-7 days after menstruation rather than complete prevention.

What it means for you

Rimegepant appears promising for preventing migraines around your period and was well tolerated. However, this was a very small case series with only 5 women and no control group. Some women's migraines were delayed rather than prevented. Much larger randomized controlled trials are needed to confirm these findings.
