Between 5% and 8% of women have moderate-to-severe symptoms that cause significant problems with work, relationships, and daily life. These are called PMS or PMDD. PMDD is the most severe form.

Systematic review, 2024

Do women with PMS or PMDD have disrupted body clocks and sleep?

Yes, the evidence so far suggests women with PMS/PMDD tend to have lower melatonin, higher nighttime body temperature, and worse sleep quality, but many other measures are mixed or show no difference, so this is not fully settled.

Case-control study, 2021

Can TSH level and premenstrual spotting help diagnose endometriosis without surgery?

High TSH levels and premenstrual spotting might help identify endometriosis, though neither marker alone is definitive. A diagnosis still typically requires surgery to confirm.

Randomised trial, 2021

Is low vitamin D linked to premenstrual symptoms?

Low vitamin D was linked to two specific physical symptoms, breast tenderness and body aches, but not to emotional PMS symptoms. The evidence is not yet definitive.

Study, 2020

Does Tazalok work better than Vitex for treating PMS?

After 3 months, Tazalok improved hormone levels more than Vitex, with bigger improvements in depression and pain. Since women were not randomly assigned to groups, we cannot be completely certain about these differences.

Survey study, 2018

Doctors should use self-report tools combined with diagnostic criteria and rule out other conditions. Treatment options include exercise, diet changes, antidepressants, anti-anxiety medications, and ovulation-suppressing drugs.

Randomised trial, 1996

Are hormone rhythms different in women with PMDD, and are they corrected by partial sleep deprivation?

Women with PMDD had higher prolactin levels and earlier TSH peak timing than normal women. Partial sleep deprivation changed these hormones, but these changes did not appear to explain how sleep deprivation helped PMDD symptoms. The evidence is moderate but based on a small study.

Review, 1994

Which hormones change across the menstrual cycle?

Body temperature and adrenaline-related chemicals change predictably. Prolactin and serotonin may change around ovulation. Thyroid hormones and cortisol appear stable.

Non-randomised trial, 1993

A small number of women with PMS had obvious thyroid disease (10.5%), and a larger number had subtle thyroid abnormalities (30%), but thyroid hormone treatment was no better than placebo for PMS overall.

Cohort study, 1991

Does tubal sterilization cause premenstrual syndrome?

No. A study of 78 women with PMS found no difference in symptom severity or hormone levels between those who had tubal sterilization and those who didn't, suggesting PMS is probably not caused by the procedure.

Survey study, 1990

What do we know about what causes PMS and how common it is?

A literature review found PMS affects 30 to 40% of women, especially those who work outside the home, have had multiple children, or use alcohol. Many theories about causes exist, but none are proven.
